First Ask: Is This Actually a hOP?

Not every problem is a Human Operating Problem. If a server is physically down, the immediate problem may be technical. If an expensive platform is available but nobody uses it because expectations, training, ownership, workflow, and accountability are unclear, that is likely a hOP.

A process or SOP problem can still be a hOP when it affects how humans understand, adopt, communicate, decide, execute, or work through that process. Diagnose before choosing the operating system.

The 5 hOS™ Phases

1 ★ assessment

What is happening now? Observe the visible problem, context, humans, process, tools, patterns, assumptions, and prior attempts. Confirm whether the issue is actually a hOP before going further.

2 ★ identification

What is the real problem? Separate the symptom from the root cause. Identify the hOP that is affecting how the human or humans operate.

3 ★ activation

What changes now? Define the human choices, commitments, ownership, behaviors, and actions required to change the operating result.

4 ★ implementation

What do we practice? Turn the decision into repeatable behavior, useful workflows, support, measures, and a way of working humans can actually follow.

5 ★ reinforcement

What holds? Use feedback, accountability, observation, outcomes, and course correction to sustain what works and identify what needs recalibration.

Why Recalibration Matters

hOS™ is iterative, not one-and-done. New information, feedback, implementation, or outcomes may reveal that the original diagnosis was incomplete or wrong. That does not mean the system failed. It means the human operating system is doing what good diagnosis should do: learn and adjust.

Like a Career Doc or medical diagnosis, a symptom is not the diagnosis. Sometimes the first treatment teaches us that we need to return to assessment or identification before moving forward again.
